Bispectral Index Monitoring Improves the Cognitive Function after Desflurane –Anaesthesia in Elderly Patients Undergoing Spine Surgery

چکیده
Postoperative cognitive dysfunction is a common complication in the elderly patients receiving desflurane anaesthesia. The bispectral index score (BIS) -targeted desflurane anaesthesia may hasten the recovery and cognitive function in the elderly patients after spine surgery. Forty patients aged >60 yr., scheduled for elective spine surgery were randomly allocated to BIS targeted (n=20) and MAC targeted (n=20) groups. Perioperative changes in the end-tidal (ET) concentrations of desflurane, times to emergences from anaesthesia and extubation, Aldrete score, cortisol and norepinephrine and the Mini Mental State Examination (MMSE) for the first 3 postoperative days were recorded. Perioperative changes in heart rate and mean arterial blood pressure were similar in both groups. The patients in the BIS targeted group showed significantly lower ET concentrations of desflurane, shorter times to eye opening, to follow commands and to tracheal extubation and higher Aldrete score and 24-hour MMSE values compared with those in the minimum alveolar concentration (MAC) targeted group (p<.05). Perioperative cortisol and norepinephrine plasma levels were similar in both groups. It was concluded that BIS-targeted desflurane anaesthesia provides faster recovery and improves the cognitive function in the elderly patients after spine surgery.

Bispecteral index facilitates postoperative recovery of elderly patients undergoing abdominal surgery
Introduction: Elderly is considered from 60-65 years old, and 35% of all surgeries related to this age group. Surgical complications in these patients have a high incidence and to reduce these complications Bispectral (BIS) monitoring during the anesthesia can be used. This study was designed to investigate whether BIS monitoring improves clinical anesthetic delivery, recovery profiles in elder...

BACKGROUND This study was designed to investigate the impact of bispectral index (BIS) or A-line AAI index (based on middle-latency auditory evoked potential) monitoring on recovery times and drug consumption when compared with standard anaesthetic practice during desflurane-remifentanil anaesthesia. METHODS After having obtained approval from the institutional review board and written inform...

PURPOSE This study was designed to investigate the effect of bispectral index (BIS) monitoring on the recovery profiles, level of postoperative cognitive dysfunction, and anesthetic drug requirements of elderly patients undergoing elective orthopedic surgery with general anesthesia. METHODS Sixty-eight patients over the age of 60 were randomized into one of two groups. In the standard practic...
متن کاملEffect of nitrous oxide on bispectral index values at equi-minimum alveolar concentrations of sevoflurane and desflurane
BACKGROUND AND AIMS Bispectral index (BIS) values may be anaesthetic agent-specific, depending on their ability to suppress the electroencephalogram (EEG) signals. We carried out a prospective, randomised clinical trial to study the effect of nitrous oxide (N2O) on the BIS values at an equi-minimum alveolar concentration (MAC) of sevoflurane and desflurane. METHODS Sixty adult patients underg...

BACKGROUND The goal of this study was to compare the influence of an interscalene brachial plexus block (ISB) performed with either bupivacaine or levobupivacaine in conjunction with general anaesthesia (GA) on desflurane consumption, which was titrated to maintain the recovery profiles and postoperative analgesia while also keeping the bispectral index score (BIS) between 40 and 60 in patients...
